To participate in Portillo’s Survey, you must:
- Be a lawful US resident and at least 16 years old.
- Purchase to access the survey with a valid receipt from a Portillo visit.
- Complete the survey within four days of your visit.
- Use only one survey per receipt.
- Receive one free item voucher per receipt for each visit.
- Redeem the voucher within three months; it cannot be exchanged for cash.
- Do not combine the voucher with other offers.
- Do not be an employee of Portillo or a family member of an employee to participate.
